you can apply in form A which is given below. Following thing to done while preparing or filing Form A :-
1. Attach postal order of Rs. 10/-. But if information to be taken is lengthy attach postal order of more value.
2. Attach self addressed envelope with appropriate postal stamps affixed on it.
3. Send these documents to the P I O of the department from which information is sought.
